What you need to complete this form

You have been asked to complete this form so that Devon and Cornwall Police and/or Dorset Police can use the image/video footage captured for promotional purposes.

This is a standard form used by the force which is a requirement under the Data Protection Act. While completing the form you will be presented with the following statement

Without expectation of compensation or other remuneration, now or in the future, I give my consent to Devon & Cornwall Police using my image, video or statement (hereon referred to as ‘material’). I am aware that the material may be published externally or internally on social media, in newsletters, internal documents, on police and partners’ websites, on promotional materials, shared with partner agencies and with local and national media. Online graphic design platforms might be used to create external publications. Material will not be used for any other purpose other than what you have agreed to.

I give my permission for this material to be available for up to five years from the date on this form, after which time I will be contacted to gain consent to extend this timeframe if necessary. The material will be destroyed if no extension of consent has been requested or agreed. I understand I can contact Devon & Cornwall Police at any time and request that this material is destroyed. Personal details including email addresses, names and shoulder numbers will not be published.

We will ask you to:

• Read and agree to the above terms and conditions.
• Confirm you are the person or legal guardian of the person contained in the material.
• Provide some personal details in the event we need to contact you to renew the terms of your consent.
• Provide a recent photograph of yourself in a sutable format (.jpg, .jpeg, .png, .bmp or .exif).

What happens after you submit this form

The material will be stored in line with our alliance information security policy (j-p-015). This agreement complies with the processing of images and information under the principles of the Data Protection Act 2018 which includes General Data Protection Regulations 2018.

How do we use this information

This service stores the information you provide as you progress through the form. Form submissions for this service that are not fully completed are automatically deleted after 5 days. If you contact us about a failed online submission, with your consent, we may retrieve your incomplete submission if it is still available to us. At our discretion, we may also review and process incomplete submissions where there is a threat to life, injury or vulnerability identified within the incomplete submission.

Once your form is submitted and processed, the information is retained for 3 days by this service while the information is transferred securely to the relevant force information systems.
